Get Back Your Life Contest Rules
1. Contest Description. The 3in4 Association's "Get your life back" contest (the "Contest") is designed to encourage people to tell their personal stories about giving care to seniors and the impact that has had on their lives, and to identify particularly powerful stories to publicize in order to help bring this issue home to the American people. No purchase or donation is necessary. Judges will choose the winning entry and the prize will be issued in accordance with the following Contest Rules.

2. Contest Sponsor. The Contest is sponsored by 3in4 Association (the "Sponsor"), a Washington nonprofit corporation, 5125 Carillon Point, Kirkland, WA 98033, USA.

3. Agreement. Eligible people who enter the Contest ("Entrants") agree to these Contest Rules, the terms of which constitute a binding agreement between Entrant and the Sponsor. If you do not agree to these Contest Rules you are disqualified from participating in the Contest or winning any Prize. The Contest is void where prohibited.

4. Eligibility. The Contest is open to individuals who are legal residents of the United States and who are 18 years of age or older. Employees, interns, contractors, officers and directors of the Sponsor, as well as their immediate families, are not eligible to participate.

5. Contest Period. The Contest begins May 24, 2011 at 7:00 AM EDT and ends on December 31, 2011 at 11:59 PM EST. Entries submitted before or after the Contest Period will not be considered.

6. How to Enter. All entries must be submitted online through our official "Share your Story" page on www.3in4needmore.com in writing or in video format. Entries must consist of an original, true story in which Entrant was personally involved. Sponsor disclaims any responsibility whatsoever for submissions that are lost, delayed or damaged, regardless of the cause or identity of the person who caused the loss, delay or damage. All intellectual property and industrial property rights in any entries belonging to the Entrants' submissions will become the property of Sponsor, and will not be returned after the Contest. You grant Sponsor permission to use, copy, modify, distribute, and publicly display your Contest submission for any purpose, without any attribution or compensation to you. Entrants acknowledge and agree that Sponsor shall have no obligation to post, display or otherwise make publicly available any work submitted by Entrant.

7. Selection of Winners. Entries will be judged by a panel of judges consisting of people affiliated with Sponsor, who will choose the one story (the "Winner") that, in their judgment, most powerfully illustrates the impact on the care-giver or their families of giving care to seniors. Decisions of the judges are final, binding and not subject to any review or appeal whatsoever. The panel of Judges will also select and recognize three "Finalists" for publication; however, Finalists will not receive prizes. The Sponsor will announce the Winner and Finalists on or before December 31, 2011. Sponsor reserves the right to select other Winners or Finalists in the event any Winner or Finalist is disqualified for any reason.

8. Prize. The Winner will receive a prize which Sponsor has denominated a "Caregiver Makeover" consisting of a package of goods and services (the "Prize") including:
• A week's worth of health and relaxation treatments
• An in-home safety assessment and "elderproofing" makeover
• One-on-one coaching sessions with elder care expert and author Dr. Marion Somers
• Targeted strategies to address caregiving needs, and Technology to aid with a wide range of caregiving needs
Sponsor reserves the right to substitute prizes of equivalent value (as determined in its sole discretion) if any part of the Prize becomes unavailable for any reason. Entrant agrees that Sponsor's estimate of the value of the Prize is a good faith estimate only, and Entrant shall have no claim based on any different or lower assessment of the value of the Prize. Entrant shall have no right to opt to receive cash in lieu of any part of the Prize, or to require Sponsor to exchange any part of the Prize for other goods or services.

9. Publicity. Entrant agrees to participate in any media or promotional activity regarding the Contest. Regardless of whether Entrant is declared a Winner or a Finalist, Entrant hereby grants Sponsor permission to use, copy, modify, edit, distribute, and publically display Entrant's Contest submissions for any purpose, such as, but not limited to, press and media communications, without any compensation to Entrant or any third party. Sponsor shall be authorized to grant permission to third parties to use, copy, modify, distribute, and publicly display Contest submissions for any purpose including commercial purposes, without compensation to Entrant or any third party.

10. General Terms. No purchase or donation is required to enter this Contest and purchasing a product or making a donation will not affect an Entrant's chances to win. Sponsor reserves the right to terminate this Contest at any time without prior notice. Any potential Winner who cannot be contacted within ten business days of attempted first notification will forfeit his or her prize. Potential prize winners may be required to sign, and return a liability and publicity release, an I.R.S. Form W-9, and provide any additional information that may be required by Sponsor. Failure to comply promptly may result in disqualification and selection of an alternate winner. Entrants certify that their submission is original, and that they are the sole and exclusive owner and rights holder of the submitted submission (or if they are not sole and exclusive owner and rights holder of the submission, that they have obtained the necessary permission to include the submission in the Contest) and that they have the right to submit the submission in the Contest. Entrant agrees not to participate in the Contest in any manner that would: (1) infringe any third-party proprietary, intellectual property, personal rights or other rights, including without limitation, privacy, publicity, copyright, trademark, patent, trade secret or any confidentiality obligation; or (2) otherwise violate applicable law. If any provision of these Rules is held to be invalid or unenforceable, all remaining provisions hereof will remain in full force and effect.

11. Indemnification. TO THE MAXIMUM EXTENT PERMITTED BY LAW, YOU, THE ENTRANT, AGREE TO INDEMNIFY SPONSOR FROM AND AGAINST ANY LIABILITY, ACTIONS, CLAIMS, DAMAGES, AND EXPENSES FOR WHICH SPONSOR WILL OR MAY BECOME LIABLE BY REASON OF OR RELATED OR INCIDENTAL TO ANY ACT, DEFAULT OR OMISSION BY YOU UNDER THESE RULES INCLUDING WITHOUT LIMITATION IN RELATION TO ANY BREACH, NON-OBSERVANCE OF THE CONTEST RULES, OR ANY OTHER ACT OR OMISSION BY YOU WHETHER NEGLIGENT OR OTHERWISE. TO THE MAXIMUM EXTENT PERMITTED BY LAW, YOU AGREE TO HOLD SPONSOR, ITS RESPECTIVE DIRECTORS, OFFICERS, EMPLOYEES AND ASSIGNS HARMLESS FOR ANY INJURY OR DAMAGE CAUSED OR CLAIMED TO BE CAUSED BY PARTICIPATION IN THE CONTEST AND/OR USE OR ACCEPTANCE OF ANY PRIZE WON.

12. Warranties of Entrant. By registering and submitting an entry, Entrant warrants and represents that (a) that the story is true; (b) that all people in the story have authorized Entrant to share the story publicly including their true names, and to submit the story as an entry in the Contest subject to all of the Contest Rules; (c) that the story is original and Entrant is not committing any copyright infringement or plagiarism; and (d) Entrant is eligible to enter the Contest.

13. Rule Amendments. Sponsor reserves the right to amend these contest rules. Amended rules will be posted on the www.3in4contest.com.
